No. Use Forma on its own, or connect iMIS or Salesforce with the Association plan. Forma collects responses and can update member records; your CRM remains your member database.
Yes. Upload a Word document or PDF, or paste a link to the form you already use. Forma rebuilds it so you can review and edit the text, apply your branding, and publish. Editing starts with your content rather than a drag-and-drop canvas.
Forma collects the registration or renewal details. It does not process payments or provide a checkout. Keep collecting fees through your existing payment tool.
No. Report publishing is off by default. Your team reviews the findings and decides whether to publish a shareable link. Forma warns you before identifying information goes onto a published report.
Yes. Export your responses as CSV at any time, including on Free. There is no export fee. Signed webhooks are available if you want to send responses to another system as they arrive.
The trust centre includes a privacy impact assessment, a threat and risk assessment, and a security questionnaire. Forma is not SOC 2 certified. Hosting is in Canada; if you require storage in another country, Forma will not meet that requirement.
