Anonymous, and the form says so
A compensation survey only works if people believe it is anonymous. In most tools, anonymous is a checkbox that hides the name in the export and keeps everything else.

What Forma does
In Forma it is one switch in the form’s settings, and the server enforces it: no name, no location, no device, nothing that identifies the respondent is recorded at all. The published form says so in one line, before the first question.
